Labor Day Entry Fee - $1,000 (U.S. Dollars)
November Entry Fee - $750 (U.S. Dollars)
Basic Requirements:
Travel permits for all teams when applicable
Certified USA Hockey team roster (1-T & 2-T)
Full payment to accompany application
Out-of-town teams may stay at designated tournament hotels

2008/2009 TOURNAMENT SERIES TEAM APPLICATION
The Coyotes Amateur Hockey Association (CAHA) is pleased to invite you to compete in one of our Host Tournament Series events. Teams could compete from Friday morning through early Sunday afternoon (or Monday afternoon on Holiday Weekends). Special hotel rates have been negotiated with area hotels close to our facility, Alltel Ice Den in Scottsdale, Arizona.
STAYING IN ONE OF OUR DESIGNATED HOTELS IS NOT REQUIRED. A committee will make all final team selections. Teams accepted will be provided contact information for hotel rates and contact names. No tournament application will be considered unless accompanied with the complete entry fee. Tournament has limited capacity, and no team spots will be held or guaranteed until final notification of acceptance takes place.Association/Organization: ___________________________________________
